Преждевременные роды – серьезное осложнение беременности, которое является ведущей причиной младенческой заболеваемости и смертности. Fusobacterim nucleatum, являясь представителем нормальной микробиоты полости рта, в условиях беременности играет важную роль в развитии гингивита и пародонтита, что может иметь последствия в виде преждевременных родов.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В качестве основных электронных баз данных были использованы PubMed/MEDLINE, Scopus и e-library. Дополнительные источники литературы были получены из библиографий соответствующих обзоров и консенсусных отчетов. Единого ограничения по дате начала поиска не устанавливалось. Поисковые стратегии включали термины, относящиеся к заболеваниям пародонта («пародонтит», «гингивит», «пародонтальная инфекция»), исходам беременности («преждевременные роды»), «недоношенность», «низкий вес при рождении», «мертворождение», «хориоамнионит», микробиологии полости рта и плаценты («микробиом полости рта», «интраамниотическая инфекция»), Fusobacterium nucleatum («Fusobacterium», «F. nucleatum», «FadA», «Fap2», «полимикробные взаимодействия»).</p></sec><sec><title>Результаты</title><p>Результаты. F. nucleatum приобретает возможность активно размножаться, превышая нормальный уровень своего присутствия в тканях пародонта до 10 000 раз в период беременности за счет иммунологических, эндокринных и других изменений, происходящих в организме женщины, и вызывать воспалительные заболевания пародонта. Бактерия обладает факторами вирулентности, в первую очередь адгезинами, способствующими ее гематогенному распространению из полости рта и усилению системного воспаления. Экспериментально подтверждено, что F. nucleatum может вызывать хориоамнионит, а также, за счет повышения уровня простагландинов, стимулировать родовую деятельность и приводить к преждевременным родам. Эффективная нехирургическая пародонтальная терапия на ранних этапах (при планировании беременности и в первом триместре) способствует снижению частоты преждевременных родов.</p></sec><sec><title>Заключение</title><p>Заключение. Комплексное обследование женщины на этапе планирования беременности и своевременное лечение стоматологических заболеваний способствует предотвращению развития акушерских осложнений. Профилактический подход к проблеме преждевременных родов, ассоциированных с F. nucleatum, требует внимания и дальнейшего изучения со стороны акушеров, стоматологов и микробиологов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Preterm birth is a serious pregnancy complication and remains a leading cause of neonatal morbidity and mortality. Fusobacterium nucleatum, a member of the normal oral microbiota, has been implicated during pregnancy in the development of gingivitis and periodontitis, which may, in turn, contribute to adverse pregnancy outcomes, including preterm birth.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The primary electronic databases searched were PubMed/MEDLINE, Scopus, and eLIBRARY. ru. Additional sources were identified through reference lists of relevant reviews and consensus reports. No restriction on the start date of the search was applied. Search strategies included terms related to periodontal diseases (“periodontitis,” “gingivitis,” “periodontal infection”), pregnancy outcomes (“preterm birth,” “prematurity,” “low birth weight,” “stillbirth,” “chorioamnionitis”), oral and placental microbiology (“oral microbiome,” “intra-amniotic infection”), and Fusobacterium nucleatum (“Fusobacterium,” “F. nucleatum,” “FadA,” “Fap2,” “polymicrobial interactions”).</p></sec><sec><title>Results</title><p>Results. During pregnancy, F. nucleatum acquires the ability to proliferate actively. As a result of immunological, endocrine, and other physiological changes in the maternal physiology, its levels in periodontal tissues may increase by up to 10,000-fold, contributing to the development of periodontal diseases. The bacterium possesses virulence factors—primarily adhesins—that facilitate hematogenous dissemination from the oral cavity and amplification of systemic inflammation. Experimental studies have demonstrated that F. nucleatum can induce chorioamnionitis and, through increased prostaglandin production, stimulate uterine contractions, ultimately leading to preterm birth. Early-stage non-surgical periodontal therapy (during pregnancy planning and the first trimester) has been shown to reduce the incidence of preterm birth.</p></sec><sec><title>Conclusion</title><p>Conclusion. Comprehensive examination of women at the pregnancy planning stage and timely management of periodontal and other oral diseases may contribute to the prevention of obstetric complications.
