Продолжительность жизни людей с ВИЧ возросла из-за применения специфической антиретровирусной терапии (АРТ). В настоящее время одна из актуальных медицинских проблем для этого контингента – остеопороз. Восстановление иммунитета на фоне АРТ приводит к устойчивой воспалительной реакции в организме, выбросу цитокинов, активации остеокластов. Остеопороз (далее ОП) часто становится причиной осложнений в пародонтологии и имплантологии. Существенная потребность ВИЧ-инфицированных в этих видах стоматологической помощи и распространенность пародонтитов, в том числе генерализованных форм, также обуславливают важность данной проблематики.</p></sec><sec><title>Цель</title><p>Цель. Выявить общие звенья патогенеза системной (ОП) и местной (челюстных костей при пародонтите) остеорезорбции, основные методы диагностики, лечения и профилактики ОП при ВИЧ-инфекции, его влияние на результаты имплантологического лечения адентий у этого контингента.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Анализ публикаций в научной периодике по тематике «остеопороз + патология пародонта + ВИЧ/СПИД» за последнее десятилетие (PubMed, Medline, Elibrary).</p></sec><sec><title>Результаты</title><p>Результаты. Этиология нарушений костной структуры многофакторна: хроническое воспаление, прямое влияние ВИЧ на костные клетки, воздействие АРТ (ингибиторы вирусной протеазы) на ремоделирование и метаболизм костной ткани. Планирование конкретных терапевтических мероприятий должно быть индивидуализированным и направлено на основные звенья патогенеза остеорезорбции (остеопротекторы, противовоспалительные, бисфосфанаты, препараты кальция, витамина D).</p></sec><sec><title>Заключение</title><p>Заключение. Эффективность бисфосфонат-терапии ОП и DEXA-скрининга для контроля риска остеорезорбции у этого контингента позволяют говорить о клинически оправданном имплантационным методе лечении адентий при ВИЧ-инфекции. Актуальные проблемы ОП и патологии пародонта в связи со старением ВИЧ-больных должны рассматриваться как связанные между собой. Для них решающим становится планирование конкретных диагностических (DEXA, биохимический скрининг) и терапевтических (реминерализуюшие препараты, остеопротекторы и пр.) стратегий с участием узких специалистов (эндокринолог, гастроэнтеролог и пр.).</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. The life expectancy of people with HIV has increased thanks to the use of specific antiretroviral therapy (ART). Currently, one of the urgent medical problems for this cohort is osteoporosis. Immune reconstitution during ART leads to a sustained inflammatory reaction in the body, the release of cytokines, and activation of osteoclasts. Osteoporosis often causes complications in periodontology and implantology. The significant need of HIV-infected people for these types of dental care and the prevalence of periodontitis, including generalized forms, also determine the importance of this issue.</p></sec><sec><title>Purpose</title><p>Purpose. To identify the common links in the pathogenesis of systemic (osteoporosis – hereinafter referred to as OP) and local (jaw bones in periodontitis) osteoresorption, the main methods for the diagnosis, treatment and prevention of OP associated with HIV infection, its effect on the results of implantological treatment of adentia in this population.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Analysis of publications in scientific periodicals on OP + Periodontal Pathology + HIV/AIDS over the past decade (PubMed, Medline, Elibrary).</p></sec><sec><title>Results</title><p>Results. The etiology of bone structure disorders is multifactorial: chronic inflammation, direct impact of HIV on osteocytes, effect of ART (viral protease inhibitors) on bone metabolism and remodeling. The planning of specific therapeutic interventions should be individualized and aimed at the key links in the pathogenesis of osteoresorption (osteoprotectors, antiinflammatory agents, bisphosphonates, calcium supplements, vitamin D). </p></sec><sec><title>Conclusion</title><p>Conclusion. The effectiveness of the bisphosphonate therapy of OP in this cohort and DEXA screening to control the risk of osteoresorption suggest a very likely clinically justified implantation method for the treatment of tooth loss in HIV infection. Topical issues of OP and periodontal pathology in connection with the aging of HIV patients should be considered as related. For them, the planning of specific diagnostic (DEXA, biochemical screening) and therapeutic (remineralizing agents, osteoprotectors, etc.) strategies involving narrowly specialized doctors (endocrinologists, gastroenterologists, etc.) becomes a decisive.</p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>остеопороз</kwd><kwd>ВИЧ</kwd><kwd>имплантологическое лечение</kwd><kwd>заболевания пародонта</kwd></kwd-group><kwd-group xml:lang="en"><kwd>osteoporosis</kwd><kwd>HIV</kwd><kwd>implant therapy</kwd><kwd>periodontal diseases.</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Рассохин В.
