Целью нашего исследования явилась разработка алгоритма выбора эффективного и безопасного местного обезболивания у пациентов с артериальной гипертензией в условиях амбулаторного стоматологического приема.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Исследование проводилось в лаборатории клинико-функциональных исследований МГМСУ им. А.И. Евдокимова с оценкой электровозбудимости (мкА) и микроциркуляции (М, п.е.) пульпы интактных зубов пациентов с АГ до применяемой анестезии и через 5, 10, 15, 30 и 60 минут. Использовали 4% раствор артикаина без вазоконстриктора и с минимальным его содержанием – 1:200 000 и 1:400 000, а также 3% раствор мепивакаина. Безопасность используемого местного обезболивания оценивалась непрерывным мониторированием гемодинамических показателей пациента.</p></sec><sec><title>Результаты</title><p>Результаты. Изучение инфильтрационной анестезии 4% раствора артикаина без эпинефрина на верхней челюсти и фронтальном отделе нижней челюсти продемонстрировало неглубокий анестезирующий эффект. Использование концентрации вазоконстриктора 1:400 000 и 1:200 000 в 4% растворе артикаина повышает глубину и длительность анестезии от 20 до 30 минут соответственно. Изучение функциональных показателей пульпы интактных зубов у пациентов с АГ при применении 3% раствора мепивакаина, введенного у нижнечелюстного отверстия, показало наличие динамики изменений электровозбудимости, но не гемомикроциркуляции. Анализ данных, полученных в ходе непрерывного мониторирования гемодинамики пациента, показал отсутствие изменений показателей АД, ЧСС, SpO2 при использовании всех изучаемых способов и препаратов местного обезболивания.</p></sec><sec><title>Заключение</title><p>Заключение. Проведенный анализ критериев прогнозирования эффективного и безопасного местного обезболивания позволил обосновать его выбор у пациентов с артериальной гипертензией в условиях амбулаторного стоматологического приема. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. To develop the algorithm for a safe and effective local anesthesia in dental outpatients with arterial hypertension.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study was conducted in the laboratory of functional and clinical studies of Moscow State University of Medicine and Dentistry. Electric pulp testing (µA) was performed and pulp microcirculation (PU) was assessed in the intact teeth of patients with hypertension before and 5, 10, 15, 30 and 60 minutes after the administration of local anesthesia. We used 4% articaine solutions without a vasoconstrictor and with its minimal concentration 1:200 000 and 1:400 000, and 3% mepivacaine solution. The safety of the administered local anesthetic was assessed by the continuous hemodynamic monitoring.</p></sec><sec><title>Results</title><p>Results. 4% articaine solution without epinephrine had a shallow anesthetic effect in the maxilla and anterior mandible. 1:400 000 and 1:200 000 vasoconstrictor concentrations in 4% articaine solution increase the depth and duration of the anesthesia from 20 to 30 minutes respectively. Changes in the pulp sensibility but not in blood microcirculation were demonstrated by the functional parameters of the intact dental pulp in patients with hypertension after the administration of 3% mepivacaine solution at the mandibular foramen. The continuous hemodynamic monitoring data showed no changes in arterial blood pressure, heart rate, oxygen saturation on administration of either of the studied local anesthetic solutions or techniques.</p></sec><sec><title>Conclusion</title><p>Conclusion.
