Современные исследования показывают, что сочетанное воздействие никотина и кофеина оказывает негативное влияние не только на слизистую оболочку полости рта, но и на организм в целом. Целью исследования явилось изучение сочетанного воздействия кофе и курения на состояние тканей пародонта.Материалы и методы. Проведено стандартное стоматологическое обследование 60 пациентов, средний возраст исследуемых групп составил 22,1 ± 0,3 года. В первую группу вошли пациенты, употребляющие кофе, не курящие, во вторую группу – пациенты курящие, не употребляющие кофе, и в третью – курящие и употребляющие кофе. Всем пациентам определялись гигиенические и пародонтологические индексы: папиллярно-маргинально-альвеолярный (PMA) в модификации Parma, индекс кровоточивости Мюллемана в модификации Коуэлл (SBI), упрощенный индекс гигиены полости рта по Грину – Вермиллиону (OHI–s). Микроциркуляцию в тканях пародонта изучали с помощью метода ультразвуковой доплерографии. Для изучения скорости слюноотделения, pH и вязкости слюны по методике Рединовой Т. Л., Поздеева А. Р., измеряли артериальное давление и частоту сердечных сокращений.Результаты. Употребление кофе в сочетании с курением сигарет оказывает синергический эффект на сердечнососудистую гемодинамику общего кровотока (АД, ЧСС), однако при изучении локализованной трофики в тканях пародонта отмечено, что значительное увеличение микроциркуляции возникает при изолированном употреблении кофе. При этом после курения наблюдается реверсивный эффект в трофическом обеспечении в тканях пародонта, а именно в его снижении, а показатели общего кровотока наоборот увеличиваются после курения. Также во всех исследуемых группах наблюдается увеличении скорости саливации и снижение вязкости и pH слюны, наибольшее изменения данных показателей зарегистрированы в группе комбинированного взаимодействия кофе и курения.Заключение. Сочетанное влияние употребления кофе и курения оказывает сдерживающий эффект в микроциркуляции тканей пародонта, однако при их комбинации наблюдаются отличительные характеристики параметров ротовой жидкости, что может пагубно сказываться на стоматологическом статусе пациентов.</p></abstract><trans-abstract xml:lang="en"><p>Relevance. Modern studies demonstrate that the combination of smoking and coffee drinking adversely affects the whole body, not only the oral mucosa. The study aimed to investigate the impact of coffee consumption and smoking in the oral mucosa and periodontal tissues.Materials and methods. We examined 60 dental patients, mean age of 22.1 ± 0.3 years old. The first group included nonsmoking coffee drinkers, the second – non-coffee-drinkers-smokers, the third – smokers- coffee-drinkers. In all patients, we determined hygiene and periodontal indices: PMA (papillary-marginal-attached) index Parma, Muhlemann sulcus bleeding index (SBI) modified by Cowell, simplified Green-Vermillion Oral Hygiene Index (OHI-s). The study examined microcirculation in periodontal tissues using Doppler ultrasound. We took patients’ blood pressure and heart rate to study salivary flow rate, pH and viscosity according to the Redinova-Pozdeev technique.Results. The combination of coffee consumption and smoking affects the dynamics of the blood flow (BP, HR). The study of periodontal tissue blood supply revealed a significant increase in microcirculation in non-smoking coffee drinkers. Smoking negatively affected periodontal blood supply, namely periodontal circulation decreased, while general circulation parameters increased after smoking. In all studied groups, salivary flow rate increased, and saliva pH and viscosity decreased. The third group exhibited the most significant changes in these parameters.Conclusion.
