У пациентов с сопутствующей соматической патологией в связи с нарушением гомеостаза полости рта снижается барьерная резистентность тканей пародонта, что приводит к повышению тяжести воспалительных заболеваний пародонта и обуславливает необходимость взаимодействия врачей-стоматологов и врачей-интернистов, направленного на выработку единых подходов к лечению и менеджменту таких пациентов.Цель исследования – изучить пародонтологические, гигиенические индексы полости рта, минеральный состав ротовой жидкости у стоматологических пациентов с сопутствующей соматической патологией.Материалы и методы. Обследованы 63 стоматологических пациента с сопутствующими соматическими патологиями: ишемическая болезнь сердца, хронический гастрит, хронический нефрит, хронический синусит. Проведено определение гигиенического состояния полости рта по индексу гигиены Грина – Вермильона (ОНI-S) и состояние пародонта по пародонтальному индексу ВОЗ CPITN и индексу РМА. Определение содержания кальция, магния, цинка и меди в ротовой жидкости осуществляли методом электротермической атомно-абсорбционной спектрометрии с помощью спектрометра с графитовой печью «КВАНТ.Z1».Результаты. Статистически значимое снижение гигиены полости рта на фоне выраженного воспаления пародонта установлено у пациентов с ишемической болезнью сердца и хроническим нефритом. Анализ минерального состава ротовой жидкости показал, что у пациентов с ишемической болезнью сердца отмечается снижение уровня кальция, магния и цинка. При хроническом нефрите установлено снижение в ротовой жидкости меди и цинка. У пациентов с хроническим гастритом отмечается снижение в ротовой жидкости уровня магния и меди, а у пациентов с хроническим синуситом – снижение меди.Выводы. На стоматологическом приеме при проведении лечения и назначении средств профилактики необходимо учитывать наличие у пациентов соматических заболеваний и минеральный состав ротовой жидкости.</p></abstract><trans-abstract xml:lang="en"><p>Relevance. In patients with comorbidities, the barrier resistance of periodontal tissues decreases due to oral homeostasis damage, which increases the severity of inflammatory periodontal diseases and needs the interaction of dentists and internists to develop common approaches to such patients' treatment and management.Aim. The study aimed to study the periodontal and oral hygiene indices and the oral fluid mineral composition in dental patients with comorbidity.Materials and methods. The study examined 63 dental patients with comorbidities: coronary artery disease, chronic gastritis, chronic nephritis, and chronic sinusitis. The Green-Vermillion oral hygiene index (OHI-S) detected the oral hygiene status, and the CPITN index of the WHO and the PMA index revealed the periodontal status. Electrothermal atomic absorption spectroscopy using QUANTUM.Z1 graphite furnace atomic absorption spectrometer determined the amount of calcium, magnesium, zinc and copper in the oral f luid.Results. Patients with coronary artery disease and chronic nephritis demonstrated a statistically significant decrease in oral hygiene associated with severe periodontal inflammation. Analysis of the oral fluid mineral composition showed that patients with coronary artery disease had decreased calcium, magnesium and zinc levels. The chronic nephritis patients had a copper and zinc decrease in the oral fluid. In patients with chronic gastritis, there was a lack of magnesium and copper in the oral fluid. The patients with chronic sinusitis showed a decrease in copper.Conclusions.
