Для достижения оптимальных параметров десны вокруг дентальных имплантатов на сегодняшний день проводятся операции по мягкотканной аугментации с применением аутотрансплантатов и ксеногенных заменителей. Стоит отметить недостаток клинических исследований, направленных на сравнительный анализ послеоперационного состояния пациентов и параметров эстетики.</p></sec><sec><title>Цель</title><p>Цель. Оценка клинических преимуществ и недостатков применения коллагенового матрикса в сравнении с аутогенными трансплантатами при аугментации мягких тканей в области дентальных имплантатов.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. 32 пациента с дефицитом толщины мягких тканей в области планируемой установки дентального имплантата были разделены на две группы: в 1-й операция проводилась с применением субэпителиального соединительнотканного трансплантата (CСТ), во 2-й группе использовали ксеногенный коллагеновый матрикс (КМ). В послеоперационном периоде оценивались: продолжительность оперативного вмешательства, выраженность послеоперационной боли (ВАШ-шкала) и отека, количество принимаемых НПВС в сутки, качество жизни (OHIP-14), «розовая эстетика» (PES).</p></sec><sec><title>Результаты</title><p>Результаты. Продолжительность операции в 1-й и 2-й группах составила 30,31 ± 6,98 (СО) и 26,31 ± 6,81 (СО) минут соответственно (p = 0.111). Пациенты 1-й группы (ССТ) отмечали более выраженную послеоперационную боль. В 1 сутки после операции среднее значение составило 3,06 ± 1,73 (СО) в 1-й группе и 1,94 ± 1,48 (СО) во 2-й группе (p = 0,002). Пациенты 1-й группы на 1 и 3 сутки после операции чаще принимали НПВС: 2.00 (Q1 1.00, Q3 3.00) и 1.50 (Q1 0.00, Q3 2.00) соответственно, чем пациенты 2-й группы: 1.00 (Q1 1.00, Q3 2.00) и 0.00 (Q1 0.00, Q3 1.00). Более выраженный коллатеральный отек на 1 и 3 сутки наблюдался у пациентов 2-й группы (p &lt; 0.001). Статистически значимой разницы в снижении качества жизни между пациентами обеих групп не наблюдалось. Эстетические результаты 1-й группы превосходили результаты, полученные во 2-й группе, в отношении таких параметров, как форма альвеолярного гребня и уровень маргинальной десны. Разница в цвете мягких тканей между зоной операции и окружающими тканями отсутствовала практически у всех пациентов.</p></sec><sec><title>Заключение</title><p>Заключение. Применение КМ обеспечивает более комфортный послеоперационый период, однако приводит к более выраженному коллатеральному отеку. Использование ССТ демонстрирует лучшие результаты с точки зрения оценки эстетической составляющей проведенного лечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Today soft tissue augmentation is performed with autogenous and xenografts to achieve optimal gingival parameters around implants. It is worth noting the lack of clinical studies aimed at a comparative analysis of the postoperative condition of patients and aesthetic parameters.</p></sec><sec><title>Purpose</title><p>Purpose. The study aimed to evaluate the clinical advantages and disadvantages of collagen matrix and autografts in soft tissue augmentation around dental implants.</p></sec><sec><title>Material and methods</title><p>Material and methods. Thirty-two patients with soft tissue thickness deficiency around intended implantation formed two groups: Group 1 had a subepithelial connective tissue graft (SCTG) during surgery, and Group 2 had a xenogeneic collagen matrix (CM). In the postoperative period, we assessed the surgery duration, the severity of postoperative pain (VAS scale) and ooedema, daily NSAID dose, quality of life (OHIP-14), and pink aesthetics (PES).</p></sec><sec><title>Results</title><p>Results. The surgery duration was 30.31 ± 6.98 (SD) and 26.31 ± 6.81 (SD) minutes in Groups 1 and 2, respectively (p = 0.111). Patients in group 1 had more pronounced postoperative pain. On day one after the surgery, the mean value was 3.06 ± 1.73 (SD) in group 1 and 1.94 ± 1.48 (SD) in group 2 (p = 0.002). Patients in group 1 more often took NSAIDs on days 1 and 3 after surgery: 2.00 (Q1 1.00, Q3 3.00) and 1.50 (Q1 0.00, Q3 2.00), respectively, than patients in group 2: 1.00 (Q1 1.00, Q3 2.00) and 0.00 (Q1 0.00, Q3 1.00). Patients of Group 2 showed a more pronounced collateral oedema on days 1 and 3 (p&lt;0.001). There was no statistically significant difference in the reduction in quality of life between patients in both groups. The aesthetic results in Group 1 were better than in Group 2 by the shape of the alveolar ridge and marginal soft tissue level. There was no difference in the colour of soft tissues between the surgical area and surrounding tissues in almost all patients.</p></sec><sec><title>Conclusion</title><p>Conclusion. The CM provides a more comfortable postoperative period but, however, leads to more pronounced collateral ooedema.
